Job Description
Job title: Care Coordinator – 12 month Fixed Term Contract Role Summary: Coordinate and deliver person-centred domiciliary care, ensuring seamless service delivery, staff management, and continuous improvement of client outcomes within a 12‑month contract. Expectations: - Provide proactive, high‑quality client care and support workforce motivation. - Maintain operational efficiency, accurate records, and compliance with care standards. Key Responsibilities: - Develop, implement and adapt individualized care plans and schedules. - Manage staff allocation, resolve staffing issues, and support training and development. - Lead regular team and stakeholder meetings to promote collaboration and problem‑solving. - Liaise with clients, families, and external partners to maintain strong communication. - Supervise and ensure adherence to medication and care standards. - Prepare and maintain accurate client progress reports and operational documentation. - Plan resource deployment, assist with scheduling, billing, payroll, and on‑call duties. Required Skills: - Proven experience in domiciliary or related care settings. - Strong customer service, communication, and interpersonal skills. - Excellent organisational, logistical planning, and problem‑solving abilities. - Ability to remain calm and collaborative under pressure. - Experience in mentoring or coaching team members. - Own reliable transport. Required Education & Certifications: - Relevant experience in domiciliary care or equivalent. - No specific formal qualifications required, but NHS/CQC approved certifications (e.g., Health and Safety, First Aid) are advantageous.
