Job Description
Job Title: Senior Occupational Therapist – Frailty Role Summary: Deliver comprehensive occupational therapy assessment and intervention for frailty patients admitted through the Frailty Assessment Unit and SDEC, support MDT discharge planning, collaborate with community partners, deputise Team Leader, and mentor junior staff. Expectations: • Manage a daily clinical caseload, diagnosing and formulating individualized treatment plans. • Provide specialized therapy advice to the multidisciplinary team (MDT). • Coordinate with community partners to ensure smooth transfer of care. • Deputise for the Team Leader on clinical and managerial matters. • Lead training and supervision of new graduates, students, apprentices, and support workers. Key Responsibilities: 1. Assess patients in the Frailty Assessment Unit and Frailty SDEC area. 2. Develop and implement therapeutic interventions tailored to frailty needs. 3. Facilitate comprehensive discharge planning, prioritizing patient preferences. 4. Offer expert clinical advice to MPs, physiotherapists, therapy assistants, and discharge managers. 5. Collaborate with external community services for continuity of care. 6. Manage clinical risk and contribute to quality improvement initiatives. 7. Deputise for the Team Leader in clinical governance, resource allocation, and staff welfare. 8. Conduct training, supervision, and mentorship for junior team members. Required Skills: • Proficient in frailty assessment, intervention planning, and patient-centered therapy. • Strong communication and collaboration within multidisciplinary teams. • Leadership and delegation abilities to deputise for team management. • Training and mentoring skills for novice therapists and support staff. • Proficiency in clinical documentation, audit, and discharge planning systems. Required Education & Certifications: • Qualified Occupational Therapist with a Bachelor’s degree in Occupational Therapy (or equivalent). • Registration with the Health and Care Professions Council (HCPC). • Proven clinical experience in geriatric or frailty care (minimum 3–5 years). • Additional training in frailty or gerontology preferred.
