About the Team:

The Supported Housing Commissioning Team commission services for Lambeth people including homeless young people, young parents, children looked after, care leavers, unaccompanied asylum-seeking young people, vulnerable adults, and individuals who are rough sleeping. This post will sit within this team but is an operational role focusing on the Routes to Independence Pathway for homeless young people, children looked after and care leavers.

About the Role:

The Young People’s Pathway Manager works across both Housing and Children’s Social Care to oversee the functioning of young people’s supported housing and semi-independent living, to secure the best outcomes for our service users. The role does not work directly with young people nor involve any day-to-day contact with young people themselves. The focus is on extensive partnership working in order to ensure excellent service delivery with agencies including the Pathway providers; Housing Needs; Brokerage and Leaving Care Teams within Children’s Social Care; and the Youth Justice Service and Probation Services.

Day-to-day the Pathway Manager will manage flow into and out of the pathways, including assessing and matching referrals that have been received from colleagues in Housing and Children Social Care and working with service providers to organise suitable housing for each service user’s needs; leading on placement sustainment interventions; identifying and managing voids; tracking service user progress towards move-on, whilst identifying any barriers to move-on and finding solutions in partnership with providers and relevant partners. The Pathway Manager will also be responsible for overseeing and managing referrals into the move on services.

To be considered for interview, your CV and supporting statement will clearly evidence:

Extensive experience of working with vulnerable and/or young people.
Experience of establishing effective recording systems, and able to analyse information within those.
Experience of working with performance indicators, and able to present complex issues to a variety of audiences in written or verbal formats.
Proven experience of networking at all levels with the ability to build relationships to achieve objectives and improve processes and delivery.
Excellent time management skills with proven experience of dealing with a variety of urgent tasks, prioritising as required.
