Position: Care Coordinator

Type: Hourly contract

Compensation: $70 to $90 per hour

Location: Remote

Commitment: Flexible hours during short term project

Role Responsibilities

Design and operationalize a human led care coordination service across education, therapeutic, and medical domains.
Define care coordinator responsibilities, boundaries, escalation paths, and operating principles.
Build and refine playbooks and SOPs for intake, triage, case planning, referrals, and cross system coordination.
Establish quality controls and performance metrics for care coordination teams.
Collaborate cross functionally to ensure scalable, structured service delivery.

Requirements

Experience overseeing or operating care coordination services supporting families with complex needs.
Demonstrated ability to design operating models, scope boundaries, and documentation standards.
Experience building workflows, playbooks, and quality assurance systems.
Strong written communication skills and structured operational thinking.
Ability to work independently in a remote, fast paced environment.
